We help eligible people identify their needs, develop a plan for addressing barriers, connect with appropriate services, and coordinate support across community resources and providers.
HRSN Screening
We screen for Health-Related Social Needs that may affect a person's health and well-being, including needs related to food, housing, transportation, utilities, employment and benefits, and other essential supports..
Eligibility Assessment
We help determine whether people may qualify for available programs and services and explain the next steps in clear, understandable language.
Community Resource Navigation & Referrals
Community Health Workers help people identify appropriate resources, understand available options, and connect with community-based programs and services that address their needs.
Case Management & Care Coordination
Case Managers provide enhanced support for people who need additional coordination. We help organize services, follow up on referrals, address barriers, and support people as they work toward greater stability, independence, and well-being.
