About Rosegarden Healthcare Services
Rosegarden Healthcare Services is a community-based organization dedicated to helping individuals and families address the social needs that can affect their health, stability, and overall well-being.
We believe people deserve support that recognizes the whole person—not just one need at a time. Our approach combines personal guidance, community resource navigation, care coordination, and ongoing support.
Why Rosegarden Was Founded
Rosegarden Healthcare Services was founded by a nurse who saw firsthand that many people were struggling to access healthcare and maintain their health because essential needs outside the healthcare system were not being met.
Challenges such as transportation, food access, housing instability, difficulty understanding available services, and navigating complicated systems can create real barriers to health and well-being.
Rosegarden was created to help bridge those gaps. Instead of expecting people to navigate complex systems alone, our team helps clients understand their options, connect with appropriate resources, and coordinate the support they need.
How We Support Our Community
Community Health Workers
Community Health Workers help identify Health- Related Social Needs (HRSN), assess eligibility, provide navigation, and connect clients with appropriate community resources and referrals.
Case Managers
Case Managers provide enhanced care management and coordination for clients who need additional support. They help organize services, follow up on referrals, address barriers, and support progress toward greater stability and independence.
Person-Centered Support
Every person's circumstances are different. We listen first, understand what matters most, and help develop next steps that reflect each client's individual needs and priorities.
Community Connections
We help connect individuals and families with resources related to food access, housing, transportation, utilities, employment and benefits, and other essential social needs.
Our Mission
Our mission is to help individuals and families overcome barriers, connect with meaningful resources, and move toward greater health, stability, independence, and well-being.
Working Together
Rosegarden Healthcare Services is a contracted provider through Healthy Alliance's Social Care Network, providing enhanced Health-Related Social Needs (HRSN) Care Management, Screenings, Eligibility Assessment, and Navigation Services.
Rosegarden accepts Medicaid, and clients do not need a doctor's referral to contact us for support.
