Rosegarden connects with the client to learn about current needs and concerns.

Screening & Eligibility

Our team reviews Health-Related Social Needs and helps determine eligibility for appropriate programs and services.

Navigation & Coordination

When appropriate, we provide navigation, referrals, case management, and coordination of services based on the client's needs.

Social Care Network Provider

Rosegarden Healthcare Services is a contracted provider through Healthy Alliance's Social Care Network, providing enhanced Health-Related Social Needs (HRSN) Care Management, Screenings, Eligibility Assessment, and Navigation Services.

Rosegarden accepts Medicaid. Eligibility for specific services depends on applicable program requirements and individual circumstances.
